■Handling of Personal Information

1. Purpose of Use of Personal Information
   1. Personal information acquired through the inquiry page will be handled within the scope of the following purposes of use:
      (1) Responses to or confirmations of inquiries.
      (2) Provision of data specified by the inquiring customer.
      (3) Provision of sample products.
      (4) Collection/provision of information related to the proper use, quality, safety, or effectiveness of products.
      (5) Collection/provision of marketing information, including the actual use of products, customer needs, etc.
   2. In the case of personal information acquired through information written directly on documents by the individual to whom the personal information pertains (hereinafter, “the Individual”), the personal information shall be handled within the purpose of use scope outlined below. However, in the case a situation arises that would involve purposes of use outside the scope outlined below, ZENOGEN PHARMA shall notify the Individual and must receive the Individual's prior consent in each instance.
      (1) Sending of products and provision of sample products.
      (2) Collection/provision of information related to the proper use, quality, safety, or effectiveness of products.
      (3) Collection/provision of marketing information, including the actual use of products, customer needs, etc.
      (4) Business-related communications, sending of postal items of various types.
2. Provision of Personal Information to Third Parties
   ZENOGEN PHARMA manages personal information appropriately and does not provide personal information to third parties without acquiring the prior consent of the Individual, except in cases (1) through (5) as follows:
   (1) Cases that require the provision of specific personal information in accordance with the law.
   (2) Cases where there are difficulties in acquiring the consent of the Individual, and it is necessary to provide specific personal information in order to protect a person's life, body, or property.
   (3) Cases where there are difficulties in acquiring the consent of the Individual, and it is especially necessary to provide specific personal information in order to improve public health or to promote the healthy raising of children.
   (4) Cases in which it is necessary to cooperate through the execution of business as stipulated by law according to national functions, local public organizations, or persons who have been entrusted with the duties thereof, when there is a risk that the execution of the abovementioned business might be impeded by acquiring the consent of the Individual.
   (5) Cases in which ZENOGEN PHARMA, based on the Act on the Protection of Personal Information of Japan, provides notification of, or makes readily accessible, the required items to the Individual in advance.
3. Joint Use of Personal Information
   ZENOGEN PHARMA may jointly use personal data with the specific parties listed below.
   (1) Scope of parties for joint use: Nippon Zenyaku Kogyo Co., Ltd.
   (2) Personal data items subject to joint use: Name, company or organization of affiliation, department, position, address, telephone number, FAX number, and email address.
   (3) Purpose of use of the joint use parties: Responses to inquiries and other business communications and the improvement of products and services.
   (4) Entity responsible for the management of personal data: Zenogen Pharma Co., Ltd.
4. Entrustment of Personal Information
   ZENOGEN PHARMA may entrust personal information to business partners to the extent necessary to achieve the purpose of use. In such cases, business partners with established personal information protection systems are selected, and a contract regarding personal information is signed.
5. Revisions
   Revisions may be made to the matters published herein. In the case of such revisions, ZENOGEN PHARMA shall issue an announcement regarding the content of said revisions to notify concerned parties.
